British Red Cross Care in the Home service
The British Red Cross Home from Hospital service aims to assist with the hospital discharge of patients who require short term, low level care and support that is often not recognised as the formal responsiblity of statutory agencies.
The service is co-ordinated by a paid employee, with a group of trained volunteers living in the Swindon area. We aim to: Ensure a smooth transition from hospital to home. Offer practical and emotional support. Promote independence and help rebuild confidence. Signpost to other agencies/support networks. Act as a safety net for those leaving hospital. Assist in reducing the amount of bed blocking within the hospital. Who is the service for? Home from Hospital support is available to anyone leaving GWH, Swindon, is aged 18 or over and who preferably lives on their own. Referrals are taken from medical staff, friends, family or the patient themselves.
