Community Team for Adults with Learning Disabilities support to adults and their carers, advice, information and care management for those with learning disabilities. Including setting up and arranging of care at home, in the community and through residential homes. Supporting individuals by sign posting them to activities/work opportunities/education in the community or day services where appropriate.
